Older Adults Support Team

assisting elders stablize and maintain a healthy lifestyle

The Older Adult Support Team provides specialized intervention for elders, ages 60 and older at high risk of suicide. This holistic program provides in-home assessment, crisis intervention, counseling, psychiatric evaluation, follow-up, and case management with the overall goal of reducing the suicide rate among elders in Broward County.

The program’s philosophy is to intervene in situations where the older adult is experiencing life crises which place them at risk of hospitalization and/or the loss of his or her current level of independence in the community.

Through assessment and care planning, the program works to stabilize the older adult in crisis by intensive, short term counseling, and the facilitation to existing resources to maintain them in the least restrictive setting.

All services are provided 24/7 by staff members who are experienced in aging and mental health issues, including:
  •  Full Time Licensed Mental Health Therapists
The program offers individualized services that support older adults in the least disruptive manner, preserving both autonomy and dignity, such as:
  • Comprehensive Needs Assessment
  • Collaborative Care Planning
  • Elder-to Elder Peer Support
  • Transitional case management with linkage to needed services
  • Wellness & Recovery Education
  • Follow up services

There is always a staff member available to effectively help elders avoid unnecessary hospitalization.
