Mental Health On-Site

helping individuals in Assisted Living Facilities & Nursing Homes cope with life’s difficulties

Emotional and mental well-being depends on more than providing care for the body. Individuals living at nursing homes of assisted living facilities (ALFs) may find themselves trying to cope with depression, anxiety, loneliness, grief, chronic physical or mental illness, or other major problems that can hinder healthy living. The Mental Health On-site program goes beyond treating just the body; we strive to help the whole person.

This innovative program uses a holistic approach. We combine aspects of medical and mental health care to meet the needs of the individual, while providing additional support and expertise to facilities.

The Mental Health On-site team rovides an array of services including:
  • Staff Training
  • Psychiatric consultation
  • Assessment
  • Individual, family and group therapy

Mental Health On-site services are for adults 18 years and older. There is no cost to the individual for our specialized care.

Specialized Care: The Mental Health On-site psychiatrist will visit the facility weekly and communicate recommendations to staff.

Meeting Participation: The psychiatrist will attend monthly psychotropic drug meetings. Consultations at Care Plan meetings can be provided as well.

Educational Services: Training, tailored to meet the specific needs of each facility, can be provided to staff on an on-going basis. Topics include:
  • Psychosocial needs of the elderly
  • Identifying residents in need of the Mental Health On-site services
  • Depression
  • Management of aggressive and combative residents
  • Mental illness

The Mental Health On-site team provides centralized intake, full-time staff psychiatrist(s), and a part-time bi-lingual psychiatrist, enabling us to meet your needs in a timely manner

Assessment:

This includes intake evaluation, psychiatric evaluation, mental health status exam, on-going medication review, and psychological assessment.

Individualized Care Planning:

Counselors and individuals work collaboratively to outline individual goals and objectives.

Individualized Therapy:

Weekly or as-needed sessions that focus on issues identified in the individual’s care plan.

Family Therapy:

 Specialized services that provide counseling, support, education, and resource referrals to family members.

Group Therapy:

Group sessions covering a variety of topics designed to increase socialization and self-esteem.
